Cork & Kerry Living Well Programme
A Programme for Adults with Long-term Health Conditions
Delivered through the Family Resource Centre National Forum, supported by the HSE 

​​The Family Resource Centre National Forum (FRCNF) in collaboration with Health and Wellbeing HSE South West, is proud to announce the return of the Living Well programme to Cork and Kerry. This is a free, 6–week, group self-management programme for adults with chronic or long-term health conditions.

 

The partnership aims to bring this empowering Self-management programme back into communities across Cork and Kerry. Joining the initiative is Gemma Kingston, newly appointed Living Well Coordinator with FRCNF. Gemma brings extensive experience in mental health recovery, community development and holistic wellbeing, adding valuable insight and care to the programme’s delivery. Welcoming the renewed collaboration, Maeve Carmody, HSE Self-management Support Coordinator, stated: “The return of the Living Well programme reflects our commitment to accessible, community-based support for people managing chronic conditions and the HSE Southwest’s recognition of the importance of supporting people to self-manage”

About the Programme

Over the course of six weeks, participants develop practical skills and confidence to better manage their health condition. Living Well complements regular treatment and structured patient education programmes which people receive from their health care team.

​

Key features include:

  • Weekly 2.5-hour workshops for six weeks

  • In-person and online delivery

  • Facilitators with lived experience and peer support

  • A dedicated self-management book for participants

​

  • Westfield Integrated Care Centre, Ballincollig - 3 Nov to 8 Dec, 10:30 am – 1:00 pm

  • Online - 4 Nov to 9 Dec, 10:00 am–12:30 pm
