A Programme for Adults with Long-term Health Conditions

This report provides an accessible overview of the Living Well Programme, which was rolled out across six Community Healthcare Organisations with support from Sláintecare funding in 2019. It presents key data on participant outcomes, highlighting significant improvements in quality of life, physical activity, confidence in self-managing health, and reduced use of GP services.

Despite challenges such as the COVID-19 pandemic, the programme was successfully adapted for online delivery. The report also captures valuable learning from implementation and the added benefits of peer support among participants.​
